Who qualifies for an at-home eye exam?

Any adult with a diagnosis of diabetes.

Is this covered by insurance?

Yes, almost all insurances cover this type of exam but always double check with your insurance carrier if you have any questions.

How do we schedule this?

Use the scheduling form above. It is HIPAA compliant and secure. You get to choose the time and date and then we’ll be there. It is that easy.

Who will be performing and reading the results?

The exam will be performed by either a paramedic or nurse practitioner. The machine used is a Welch Allyn RetinaVue 700. The images are read by an ophthalmologist. The results are sent to your primary care provider and/or insurance carrier.

How often should someone have this done?

Once a year.

What do I need to do to prepare for an at-home eye exam?

  • Please all dogs & cats put away during the exam.

  • Have your insurance card available.

  • Masks are required for anyone who will be in the room.

  • Prepare a room that you can sit in for 5 minutes.

    • The room:

      • Should be as dark as possible with the blinds closed and lights off.

      • Could be a living room, bathroom or bedroom. It’s your home, you decide!

How can we help meet your value-based care measures?

As a mobile service provider (not a primary care provider), we help support primary care provider’s value-based care strategies and meeting their care measures.

In value-based healthcare, one of the measures that primary care practices continue to struggle with is diabetic retinopathy screening for patients with an active diagnosis of diabetes.  We want to remove the barriers preventing patients from having this performed, by coming bring mobile screening to their home, work, school, etc.

If needed we can also perform annual required laboratory examinations, as well as point of care hemoglobin A1C.    

How Do We Get Results?

Retina scans are performed with Welch Allyn’s RetinaVue 700 Imager.  The scan is captured live and transmitted to an ophthalmologist for diagnostic interpretation. The results and report are sent to the primary care provider satisfying the requirements.

How Do We Schedule?

Schedule by faxing us an order and we’ll contact the patient or online at up above with the patient. It is HIPAA compliant and secure.

FAQs for Clinics

  • Anyone with an active diagnosis of diabetes and without an active diagnosis of diabetic retinopathy.

  • All services are billed to the patient’s insurance like any traditional clinic would. There are no added fees for mobile retina scan. If the patient only needs mobile phlebotomy, there is a $40 mobile service fee.

  • When we bill the insurance carrier for the service, using a CPT® code, the measure is met showing that the screening was performed for the patient. Additionally, as part of your value-based care measure workflow, you can either:

    • Upload or send a copy of the report to the payer; or

    • Using one of the Category II CPT® Codes below, on a claim you can report the results from the screening:

    o 3072F – Diabetes Eye Exam WITHOUT evidence of Retinopathy in Prior Year

    o 2022F – Dilated retinal eye exam with interpretation by an ophthalmologist WITH evidence of retinopathy.

    o 2023F – Dilated retinal eye exam with interpretation by an ophthalmologist WITHOUT evidence of retinopathy.

    o 2026F – Eye imaging validated to match diagnosis from 7 standard field stereoscopic photo results WITH evidence of retinopathy.

    o 2033F – Eye imaging validated to match diagnosis from 7 standard field stereoscopic photo results WITHOUT evidence of retinopathy.

  • • Please all dogs & cats put away during the exam.

    • Have their insurance card available.

  • Under clinical guidelines and value-based care measures, patients should be screened yearly. Patients with an active diagnosis of diabetic retinopathy no longer require screening, however, should be actively monitored and managed by an ophthalmologist.

  • The exam will be performed by either one of our providers or clinical technicians. The results & report are sent to the ordering primary care provider and/or insurance carrier.

  • No! We are primarily an urgent care. We’ve always recommended patients keep their PCP and use us when they can’t get into them. We do not want to take away from your practice. We want to help the patient by getting their necessary screenings, PCP offices by meeting their metrics.

We are not a replacement for an ambulance. We are a mobile urgent care servicing Boise and the surrounding areas. If your issue is a medical emergency, pick up the phone and dial 911 or go to your nearest emergency room.

We are not equipped to treat chest pain/pressure, shortness of breath, weakness, eye injuries, uncontrolled bleeding, severe abdominal pain, altered mental status, head injuries, etc.
